Have this information ready

ID number or last four of SSN

Your Ohio driver license or state ID number, or the last four digits of your Social Security number.

Current Ohio address

Your residential address determines your county board and precinct.

Date of birth

The form collects identifying details; no extra documents are needed just to register.

Prior address if updating

If you moved, have your old and new addresses ready so both records get updated.

At the counter or online?

At an official office

Tell official staff during any qualifying transaction. They complete the form with you and forward it to your County Board of Elections.

Online

Ohio's Secretary of State offers online voter registration using an Ohio driver license or state ID. Both routes reach the same board.

At the visit

Simple steps

  1. Tell the clerk you want to register or update when you check in.
  2. Complete the Voter Registration / Change of Address Form.
  3. Review and sign. Confirm your address is correct.
  4. Watch for the confirmation notice or voter card from your board of elections.

Avoid a return trip

  • Do not wait past Ohio's 30-day pre-election registration deadline.
  • Do not forget to update after a move. An old address can send you to the wrong precinct.
  • Do not assume a license renewal updates your registration. You must opt in.
